ABDOMINAL SACRAL UTEROPEXY – AN ALTERNATIVE
APPROACH TO UTERINE PROLAPSE
Drs. Stephanie Cogan, Matthew Barber
The traditional surgical treatment for symptomatic uterovaginal prolapse
involves hysterectomy. The abdominal sacral uteropexy serves as an alternative
approach for uterine prolapse in those who desire preservation of their
reproductive organs. We demonstrate an approach that is a modification of the
traditional abdominal sacral colpopexy with preservation of the uterus. This is
performed with utilization of the uterosacral ligaments of the prolasping uterus
as well as the posterior cervix for placement of the mesh. As with the abdominal
sacral colpopexy, the mesh is anchored to the anterior longitudinal ligament of
the sacral promontory in a tension-free fashion. Other areas of pelvic organ
prolapse are addressed as well in order to achieve a satisfactory end result.
